Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotation once per month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and implement individualized treatment plans based on physician orders
Instruct patients and families in the use of therapeutic appliances and home exercise programs
Coordinate care with physicians, nurses, and other healthcare professionals
Maintain accurate clinical records and progress summaries
Supervise home health aides when involved in patient rehabilitation
Participate in case conferences and peer consultation

Qualifications

Active California Physical Therapist license
Minimum of 2 years of experience as a PT in an acute, home care, or rehab setting
Current BLS certification
Excellent oral and written communication skills
Strong organizational skills
Ability to work independently in a home health environment

Knowledge & Skills

Knowledge of home health regulations and Medicare Conditions of Participation
Proficiency in physical therapy assessment and treatment techniques
Ability to manage a diverse caseload across multiple territories
Comfortable with electronic documentation and timesheet submission
