Roles & Responsibilities

Provide personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop individualized care plans
Guide patients and families through exercises, mobility training, and use of therapeutic equipment
Monitor progress and communicate updates to physicians while documenting outcomes
Collaborate with home health aides and coordinate additional support services as needed
Educate caregivers on safe techniques to support ongoing rehabilitation at home

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment planning
Patient and family education techniques
Documentation and clinical reporting best practices
Understanding of Medicare and Medi-Cal regulations
Ability to work independently while collaborating with care teams
