Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments in patients' homes across the assigned territory
• Complete accurate OASIS assessments, recertifications, and all required documentation in a timely manner
• Manage a productive caseload and meet visit-based productivity expectations (including the 27-point weekly target when applicable)
• Travel safely and efficiently between patient homes while documenting visit time, travel, and clinical notes
• Collaborate with interdisciplinary team members, physicians, and family caregivers to support patient goals
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Adhere to infection control protocols and be prepared to treat COVID-positive patients when required

Qualifications

• Current CA Physical Therapist (PT) license in good standing
• BLS (Basic Life Support) certification
• Valid Driver's License, current auto insurance, and vehicle registration
• Required health clearances: annual TB, MMR, Varicella, Hepatitis B, TDap (within 10 years), annual flu
• Negative drug screen and recent physical/medical clearance as required
• Comfortable treating COVID-positive and PUI patients and following PPE/infection control guidance
• Willingness to travel within and occasionally outside the assigned service areas and to work weekends if needed
• Experience with home health practice and OASIS documentation highly preferred

Knowledge & Skills

• Proficient with OASIS documentation, recertifications, and outcomes-driven documentation
• Skilled in home health evaluation, treatment planning, and patient education
• Strong time management and route planning to meet visit productivity expectations
• Clear verbal and written communication for coordination with clinicians, patients, and families
• Ability to convert visit types into productivity points and understand visit-based billing/productivity models
• Comfortable working independently in community/home settings
