Contract Type: Travel Contract

Shift Type: Day shift, Monday-Friday (flexible hours based on patient visits)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct start of care visits for orthopedic and geriatric patients
Develop and implement individualized physical therapy plans
Collaborate with nurses and therapy team for comprehensive care
Document patient progress in HomeCare HomeBase (HCHB) EMR
Provide education to patients and families on home exercise programs
Manage a daily census of approximately 5 visits per day

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience required
Minimum 2 years of experience preferred but will consider less with strong home health background

Knowledge & Skills

Proficient in HomeCare HomeBase (HCHB) EMR
Knowledge of orthopedic and geriatric patient care
Excellent communication and documentation skills
Ability to work independently and manage schedule
