Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide physical therapy treatments in patients' homes, including assessments, evaluations, and plan of care
Supervise up to 2 PTAs or COTAs
Complete OASIS documentation and case management
Collaborate with RN manager and rehab team
Travel between patient homes within service territory

Qualifications

CA Physical Therapist License required
BLS certification (AHA) required
2 years experience in healthcare setting, home health preferred
Current driver's license and auto insurance
Experience with Homecare Home Base (HCHB) preferred

Knowledge & Skills

Knowledge of OASIS documentation
Strong organizational and communication skills
Ability to work independently in home environments
Time management and patient scheduling skills
