Roles & Responsibilities

Deliver personalized physical therapy services directly in patients' homes
Create customized treatment plans that help patients regain strength and mobility
Collaborate with interdisciplinary care teams to ensure seamless patient outcomes
Educate patients and families on exercises and recovery techniques
Document progress and adjust care plans to maximize results

Qualifications

Current California Physical Therapist license
BLS certification
Home health experience preferred
Strong communication and time management skills

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Ability to work independently in diverse home settings
Compassionate bedside manner
Strong documentation abilities
