Candidate Pay Rate: $59 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in the comfort of their homes
Develop and implement treatment plans that help patients regain mobility and independence
Collaborate with physicians and care teams to ensure seamless patient outcomes
Educate patients and families on exercises and techniques to support recovery
Document progress and adjust care plans to meet each patient's unique goals

Qualifications

Active California Physical Therapist license required
BLS certification required
Home health experience is a plus but not required
Strong communication and time-management skills

Knowledge & Skills

Patient-centered care approach
Ability to work independently in home settings
Knowledge of therapeutic exercise and rehabilitation techniques
Strong documentation and organizational abilities
