Roles & Responsibilities

• Conduct initial evaluations, progress notes, and care plan updates for home health patients
• Deliver skilled physical therapy interventions and maintenance programs as per state scope
• Supervise Physical Therapist Assistants and Home Health Aides in compliance with regulations
• Collaborate with physicians and interdisciplinary teams to adjust treatment plans
• Educate patients and caregivers on home exercise programs and adaptive strategies
• Prepare and submit OASIS assessments and discharge documentation accurately
• Apply ICD-10 coding for compliant documentation
• Participate in QAPI activities and staff education initiatives

Qualifications

• Currently licensed as a Physical Therapist in New Mexico
• Graduate of an accredited physical therapist curriculum
• Two years of clinical experience preferred; home health experience a plus
• Current CPR certification
• Reliable transportation, valid driver's license, and auto insurance
• Ability to work independently and exercise sound clinical judgment

Education

• Graduate of an accredited physical therapist curriculum approved by CAPTE, APTA, or CAHEA

Knowledge & Skills

• Strong patient care and clinical judgment skills
• Excellent communication and interpersonal abilities
• Proficient in documentation and EMR systems
• Time management and adaptability in varied settings
• Knowledge of Medicare, state regulations, and HIPAA compliance
