Roles & Responsibilities

• Assess and evaluate patients' functional status to create individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, strength, and independence
• Document treatment progress accurately and communicate changes to the care team
• Collaborate with physicians, nurses, and other rehab staff to coordinate patient-centered care
• Educate patients and families on home programs, safety, and prevention strategies
• Participate in discharge planning to ensure smooth transitions and positive outcomes
• Maintain compliance with facility policies and professional clinical standards

Qualifications

• Current Physical Therapist license (WV) or eligibility to obtain licensure in West Virginia
• Graduate of an accredited Physical Therapy program (DPT preferred)
• Basic Life Support (BLS/CPR) certification required
• Strong clinical assessment and manual therapy skills
• Excellent documentation and communication skills; team-oriented mindset
• Previous inpatient/rehab experience preferred but motivated new grads are welcome to apply

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable if licensed

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and balance interventions
• Knowledge of rehabilitation protocols for neurologic and orthopedic conditions
• Proficient with electronic medical records and timely documentation
• Ability to develop measurable goals and track patient outcomes
• Strong interpersonal skills for patient and family education
• Comfortable collaborating within an interdisciplinary rehab team
