Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans to restore mobility and function
• Deliver hands-on skilled physical therapy interventions, including gait training and therapeutic exercise
• Monitor patient progress, update goals, and adjust treatment plans to maximize outcomes
• Collaborate with interdisciplinary teams to coordinate care and ensure smooth transitions
• Document accurately and promptly in the patient record and communicate regularly with families
• Educate patients and caregivers on home exercise programs and injury prevention strategies

Qualifications

• Current state Physical Therapist license (MD license or eligibility to obtain)
• Active Basic Life Support (BLS) certification or willingness to obtain
• Experience in outpatient or inpatient rehab settings preferred
• Strong clinical reasoning, documentation skills, and teamwork mindset
• Comfortable working with adult and geriatric patient populations

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in Physical Therapy or related field accepted

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and gait/balance training
• Proficient with patient assessment and establishing measurable goals
• Familiarity with electronic medical records and timely documentation
• Excellent communication and patient education skills
• Ability to work collaboratively with nurses, physicians, and rehab aides
