Location: Berlin, NH

Shift Type: Rotating shifts including weekends (specific start/end times not provided) — must be willing/able to work independently on weekends

Candidate Pay Rate: $100 - $70 per hour

Roles & Responsibilities

Provide clinical pharmacy services and medication management across ICU, Labor & Delivery, Medical/Surgical and Emergency Department units
Independently manage medication therapy and make safe, timely medication decisions during weekend coverage
Collaborate with physicians, nurses and interdisciplinary teams to optimize patient outcomes
Perform medication order review, verification and reconciliation with attention to safety
Support use of pharmacy systems like Meditech and automated dispensing devices such as Omnicell
Participate in quality improvement, medication safety initiatives and formulary management

Qualifications

Active Pharmacist licensure (current or ability to obtain licensure in New Hampshire)
2-5 years hospital pharmacy experience required; previous Critical Access Hospital experience ideal
Proven ability to work independently, especially while covering weekends
Experience with Meditech preferred; familiarity with Omnicell is a plus
Strong clinical judgment, communication skills and interdisciplinary collaboration experience

Knowledge & Skills

Hospital pharmacy operations and acute care medication management
Experience supporting ICU, ED, L&D and medical/surgical patient populations
Familiarity with Meditech electronic health record workflows
Understanding of automated dispensing systems (Omnicell) and medication safety practices
Ability to prioritize, triage and make independent clinical decisions
