Roles & Responsibilities

• Evaluate residents and create person-centered occupational therapy plans that improve daily function
• Implement therapeutic interventions in a skilled nursing facility setting to help residents reach goals
• Collaborate closely with nursing, rehab team members, and family to coordinate care
• Document evaluations, progress notes, and discharge planning in a timely and professional manner
• Meet productivity expectations and contribute to a culture of quality outcomes
• Follow Continuum Rehabilitation Group policies and procedures to maintain compliance
• Represent the therapy team professionally on-site, including adhering to the black scrubs dress code

Qualifications

• Licensed Occupational Therapist or eligible for state licensure
• 1-3 years of Occupational Therapy experience preferred, SNF experience a plus
• Strong clinical reasoning and experience with documentation in post-acute care
• Ability to follow facility policies and meet productivity expectations
• Excellent communication and teamwork skills

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy experience
• Functional assessment and ADL-focused interventions
• Care planning, progress documentation, and discharge coordination
• Collaboration with interdisciplinary teams
• Time management and productivity tracking in a clinical setting
