Contract Type: Travel Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, 10:00 PM - 6:30 AM; cycle scheduling, every other weekend and holiday

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide direct patient care under the supervision of an RN
Administer medications and treatments as prescribed
Monitor vital signs and report changes in patient condition
Assist residents with activities of daily living
Document care accurately in PointClickCare EMR
Collaborate with the interdisciplinary team to ensure quality care
Maintain a safe, clean, and home-like environment

Qualifications

Active LPN/LVN license in Minnesota or compact state
BLS certification (AHA preferred)
Minimum 1 year of LPN experience in long-term care
Strong communication and teamwork skills

Knowledge & Skills

Knowledge of LTC regulations and patient care standards
Proficient in medication administration
EMR experience, especially PointClickCare
Ability to work in a fast-paced environment
Compassionate and patient-centered approach
