Roles & Responsibilities

Provide high-quality direct patient care with compassion and professionalism
Administer medications and treatments per plan of care and facility policy
Monitor and document patient status, changes, and vital signs accurately
Collaborate with RNs and interdisciplinary team members to support patient outcomes
Assist with ADLs and promote patient comfort, dignity, and safety
Participate in shift handoffs and maintain clear, timely communication

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable administering medications and basic treatments
Ability to pass background check and drug screening per facility policy
Strong teamwork mindset and clear communication skills

Education

LPN diploma or certificate from an accredited program
High school diploma or GED

Knowledge & Skills

Patient assessment and basic clinical skills
Medication administration and documentation
Familiarity with infection control and safety protocols
Electronic health record (EHR) documentation basics
Effective communication with patients, families, and care teams
Time management and prioritization in a fast-paced setting
