Roles & Responsibilities

Conduct comprehensive psychosocial assessments for patients across the lifespan to identify strengths, needs, and risk factors
Develop individualized, evidence-based treatment plans that address behavioral health needs and align with whole-person care
Provide therapeutic interventions for depression, anxiety, trauma, substance use, and adjustment disorders with compassion and clinical skill
Collaborate closely with primary care providers, nurses, and behavioral health colleagues to ensure coordinated, integrated care
Maintain timely, accurate clinical documentation and treatment progress notes in accordance with professional and organizational standards
Participate in interdisciplinary case consultations, quality improvement efforts, and care coordination activities
Connect patients and families with community resources and supports to improve access and outcomes in a rural setting

Qualifications

Master's degree in Social Work from an accredited program
Active Licensed Independent Social Worker (LISW) license in Iowa
Minimum of 2 years of clinical experience preferred
Experience working with underserved populations and/or in rural or medically underserved settings is a plus
Strong interpersonal, organizational, and clinical documentation skills
Comfort working as part of an integrated primary care/behavioral health team

Education

Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

Psychosocial assessment and clinical formulation
Treatment planning and goal-setting grounded in evidence-based approaches
Delivery of evidence-based therapeutic interventions (trauma-informed care, cognitive and behavioral approaches)
Clinical documentation and use of professional standards for record-keeping
Interdisciplinary collaboration and care coordination
Cultural humility and experience serving underserved populations
