Roles & Responsibilities

• Provide compassionate personal care to residents, assisting with activities of daily living
• Monitor and report changes in resident condition to the nursing team
• Assist with safe transfers, positioning, and mobility support
• Support residents during meals and help with feeding when needed
• Document care accurately and promptly according to facility guidelines
• Work collaboratively with nurses and therapy staff to support individualized care plans
• Maintain a clean, safe, and comfortable environment for residents
• Promote resident dignity, independence, and social engagement

Qualifications

• Valid Certified Nursing Assistant (CNA) certification (NY State preferred)
• Current CPR/BLS certification or willingness to obtain
• Ability to pass background checks and required health screenings
• Reliable, compassionate, and patient-centered approach
• Previous long-term care or skilled nursing experience preferred but not required

Education

• High school diploma or GED preferred; CNA certification required

Knowledge & Skills

• Strong interpersonal skills and resident-focused communication
• Basic clinical skills: vital signs, ADL assistance, transfers
• Infection control and safe handling techniques
• Comfortable with bedside documentation and basic charting
• Ability to follow care plans and prioritize resident safety
