Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Perform timely vital signs and report changes to the nursing team
• Support safe mobility and transfers while following proper lifting protocols
• Maintain accurate patient documentation and care records
• Follow infection control and safety procedures to protect patients and staff
• Communicate clearly with patients, families, and interdisciplinary team members to promote continuity of care

Qualifications

• Active Certified Nursing Assistant (CNA) certification (current and in good standing)
• BLS/CPR certification preferred or willingness to obtain if required
• Comfortable working in a fast-paced rehabilitation/long-term care setting
• Strong communication and teamwork skills with a patient-first attitude
• Prior CNA experience in rehab or long-term care is a plus

Education

• High school diploma or GED preferred
• Certified Nursing Assistant (CNA) credential required

Knowledge & Skills

• Skilled in assisting with ADLs including bathing, dressing, and toileting
• Proficient in taking and recording vital signs accurately
• Knowledge of basic infection prevention and patient safety practices
• Ability to observe and report patient changes promptly
• Good documentation habits and familiarity with electronic or paper charting
• Team-oriented mindset and strong interpersonal skills
