Roles & Responsibilities

• Provide thorough speech-language pathology evaluations and individualized treatment for skilled nursing residents
• Develop and implement meaningful treatment plans that support functional communication and swallowing safety
• Collaborate closely with an incredible interdisciplinary therapy team to coordinate resident care
• Document assessments, progress, and discharge planning clearly and timely per facility standards
• Meet productivity expectations and contribute to clinical efficiency (90% productivity requirement)
• Follow Continuum Rehabilitation Group (CRG) policies and procedures and uphold facility standards
• Maintain professional appearance and adhere to dress code (black scrubs)

Qualifications

• 1-3 years of SLP experience (as noted in the job posting)
• Experience working in a skilled nursing facility (SNF) is a plus
• Valid SLP credentials/licensure and any facility-required certifications (state/facility dependent)
• Comfortable working within facility policies and productivity expectations
• Reliable presence on-site to receive guaranteed hours and engage with the therapy team

Knowledge & Skills

• Knowledge of SNF practice setting and skilled nursing therapy needs
• Skilled in speech-language and swallowing assessment and intervention
• Strong documentation skills and familiarity with productivity expectations
• Ability to collaborate with nursing and rehab staff for coordinated resident care
• Professional communication skills and resident-centered care approach
