Roles & Responsibilities

• Coordinate comprehensive discharge planning to ensure safe transitions of care
• Conduct needs assessments and arrange DME and community services as needed
• Manage care coordination across the interdisciplinary team, advocating for patients
• Perform utilization review and ensure documentation meets CMS/regulatory standards
• Document thoroughly and accurately in EPIC to maintain continuity of care
• Provide patient and family education focused on post-discharge needs and resources
• Float to and support multiple hospital areas, including Colby and Pacific campuses

Qualifications

• Active RN licensure required (pending licenses accepted)
• BLS certification required
• Minimum 1 year acute care experience
• Required experience in discharge planning and care coordination
• Strong preference for candidates with diverse case management experience (Med Surg, LLOS, ED, ICU)
• Comfort with patient ratios ~1:25–30 and fast-paced acute hospital settings
• Ability to float between nearby campuses (Colby and Pacific)

Education

• Active RN licensure (required)
• BLS certification (required)

Knowledge & Skills

• Proficient with EPIC charting and electronic documentation
• Solid understanding of discharge planning and care coordination workflows
• Experience with case management and utilization review processes
• Familiarity with CMS regulations and regulatory documentation
• Skilled at needs assessment, ordering DME, and developing plans of care
• Strong communication, collaboration, and patient advocacy skills
