Shift Type: 4x10 Day shifts (four 10-hour day shifts per week), 07:30 AM - 06:00 PM, every other weekend required (specific weekdays vary) - Day shift
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate and manage safe, timely discharges — ensure patients transition smoothly to the next level of care
- Perform care coordination and needs assessments, ordering DME and creating clear plans of care
- Collaborate closely with interdisciplinary teams (including social services) to remove barriers and expedite care
- Utilize Epic to document case activity, track utilization review items, and communicate plans
- Support throughput and length-of-stay reduction efforts with focused discharge planning
- Float across hospital areas (including Colby and Pacific campuses) to meet patient and unit needs
Qualifications
- Active RN licensure required (pending licenses accepted) or MSW considered
- BLS certification required
- Minimum 1 year acute care experience (Discharge Planning/Care Coordination experience required)
- Experience with case management, utilization review, and care coordination
- Comfort working with diverse acute-care specialties (Med-Surg, LLOS, ED, ICU preferred)
- Familiarity with CMS regulations and documentation expectations
- Willingness to work every other weekend and float between units as needed
Education
- Active RN license (associate or bachelor's level acceptable) or
- MSW (Master of Social Work) — MSW candidates are considered
Knowledge & Skills
- Proficient charting and documentation in Epic
- Strong discharge planning and care coordination skills
- Experience ordering DME and developing comprehensive plans of care
- Ability to navigate utilization review processes and CMS requirements
- Effective interdisciplinary communication and patient advocacy
- Adaptability to work across multiple hospital campuses and patient populations
