Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Manage care coordination and discharge planning for a caseload of 15-20 patients per day
Conduct initial assessments and utilization review using Interqual criteria
Collaborate with interdisciplinary teams to ensure timely and safe patient transitions
Work with all payers, including uninsured and homeless populations
Document and coordinate care in Cerner EMR
Effectively reprioritize tasks throughout the day to meet patient needs

Qualifications

Active California RN license
BLS certification
Prior recent experience at a Dignity Health or CommonSpirit facility
Minimum 1 year of acute care case management or utilization review experience
Strong communication and collaboration skills

Education

Associate or Bachelor of Science in Nursing

Knowledge & Skills

Proficiency with Interqual criteria and Cerner EMR
Ability to perform both utilization review and discharge planning
Excellent organizational and reprioritization skills
Knowledge of payer requirements and community resources
