Roles & Responsibilities

• Provide resident-centered physical therapy services under the supervising therapist's plan of care to maximize functional abilities and independence
• Perform therapeutic procedures and progress treatment programs while observing precautions and contraindications
• Implement and monitor treatment plans, adjusting and communicating changes to the evaluating therapist
• Document all therapeutic interactions accurately and promptly so the whole care team stays informed
• Recommend, obtain, and instruct residents on assistive devices to improve safety and function
• Educate residents and caregivers with clear, compassionate instruction and document teaching outcomes
• Participate in interdisciplinary team meetings and collaborate to deliver coordinated care
• Maintain department cleanliness and follow infection control procedures to ensure a safe environment

Qualifications

• Associate degree required
• PT Assistant License required
• Basic Life Support (BLS) certification required
• Prior experience in long-term care or skilled nursing facility preferred
• Strong verbal and written communication skills, including presentation ability
• Proficient with documentation and Microsoft Office
• Ability to lift up to 50 pounds and perform physical tasks associated with resident care
• Commitment to resident-centered, compassionate care and teamwork

• Associate degree (required)

• Patient care and therapeutic procedures under supervision
• Treatment plan implementation and progression toward functional goals
• Clinical documentation best practices and timely record-keeping
• Interdisciplinary collaboration with nursing and rehab teams
• Infection control procedures and safe equipment handling
• Assistive device fitting, recommendation, and patient/caregiver training
• Familiarity with Medicare and Medicaid documentation and reimbursement basics
• Analytical decision making and time management in a clinical setting
