Roles & Responsibilities
- Evaluate residents and develop individualized therapeutic plans of care with clear functional goals
- Deliver therapeutic interventions and use appropriate therapeutic modalities to maximize resident independence
- Document all evaluative and therapeutic interactions accurately in the Electronic Medical Record
- Collaborate with physicians, nurses, therapists, and caregivers to coordinate care and discharge planning
- Recommend and train residents on assistive devices and ensure safe usage
- Monitor treatment effectiveness, progress interventions, and modify plans as needed
- Supervise and mentor assistant-level staff and support new staff orientation
- Provide in-service education to nursing and other departments and contribute to program development
- Maintain compliance with Medicare/Medicaid documentation and applicable state regulations
- Support department cleanliness, infection control practices, and equipment maintenance
Qualifications
- Active PT license in the state of Massachusetts (required)
- Bachelor's degree in a related field required; Master’s or Doctorate in Physical Therapy preferred
- BLS certification preferred (Basic Life Support)
- Prior experience in long-term care or skilled nursing facilities preferred
- Strong documentation skills with Electronic Medical Record experience
- Proven ability to collaborate within an interdisciplinary team and communicate effectively
- Ability to perform physical duties of the role, including lifting up to facility policy limits
Education
- Bachelor's degree required
- Master's or Doctorate in Physical Therapy preferred
Knowledge & Skills
- Patient assessment and comprehensive evaluations
- Treatment planning and implementation of therapeutic interventions
- Familiarity with assistive devices and training residents in their use
- Medicare, Medicaid, and private pay documentation requirements
- Excellent time management and clinical documentation skills
- Strong verbal, written, and presentation communication skills
- Resident-centered care approach and strong interpersonal skills
- Proficiency with Microsoft Office and electronic medical record systems
