Location: Lexington, Kentucky

End Date: 10-26-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $35 per hour

Roles & Responsibilities

Provide exceptional physical therapy services in patients' homes
Conduct comprehensive evaluations and develop treatment plans
Perform start of care, recertifications, evaluations, and daily visits
Help patients recover from injuries and improve mobility
Work autonomously with support from a dedicated clinical team 24/7
Utilize Homecare Homebase for efficient charting and documentation
Make a meaningful difference in the comfort of patients' homes

Qualifications

Active Physical Therapy Assistant license in Kentucky
BLS certification (AHA required)
Minimum 1 year of experience as a PTA
Home health experience preferred but not required
Willing to accept first-time travelers (no new grads please)
Reliable transportation and valid driver's license

Education

Associate's degree in Physical Therapy Assisting from an accredited program

Knowledge & Skills

Proficient in Homecare Homebase (HCHB)
Strong clinical assessment and problem-solving skills
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively
Compassionate and patient-centered approach to care
Knowledge of home health regulations and OASIS documentation
