Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to improve function and quality of life
• Develop and implement individualized treatment plans tailored to each resident's goals
• Collaborate with an interdisciplinary team to provide coordinated, person-centered care
• Document clinical care accurately and timely to meet facility and regulatory standards
• Participate in care conferences and family discussions to advocate for resident needs
• Contribute to ongoing quality improvement efforts to enhance therapy outcomes and facility performance

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills in evaluation and treatment techniques
• Excellent communication and documentation skills
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented with a commitment to compassionate care
