Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to improve function and quality of life
• Develop and implement individualized treatment plans that target patients' highest functional potential
• Collaborate with an interdisciplinary team to coordinate seamless, patient-centered care
• Document accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and contribute to ongoing quality improvement efforts

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills with the ability to assess and treat a variety of post-acute rehab needs
• Excellent communication skills for patient interaction and interdisciplinary collaboration
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented with a commitment to compassionate care
