Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Provide compassionate physical therapy care to patients across four rural counties
- Conduct OASIS assessments and Start of Care evaluations
- Help elderly patients recover from falls and fractures
- Manage a manageable caseload of 4-6 patient visits daily
- Document visits and track time using tablet technology
Qualifications
- Current Physical Therapist license in Indiana
- Experience with OASIS assessments preferred
- Comfortable working in rural community settings
- Strong communication and documentation skills
