Roles & Responsibilities
- Deliver personalized physical therapy care to patients in their homes
- Develop and implement individualized treatment plans that promote recovery and independence
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Educate patients and families on exercises and techniques to support ongoing progress
- Document patient progress and adjust care plans to achieve optimal outcomes
Qualifications
- Current California Physical Therapist license required
- BLS certification required
- Home health experience preferred
- Strong communication and interpersonal skills
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and care planning
- Ability to work independently in diverse home settings
- Strong organizational and time management abilities
