Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Conduct start-of-care visits especially with orthopedic patients
Develop personalized treatment plans for geriatric patients with multiple conditions
Help patients recover from joint replacements and manage chronic illnesses
Document care using the HomeCare HomeBase EMR system
Travel throughout Tehama County to deliver quality care where it's needed most

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Valid driver's license and reliable transportation

Knowledge & Skills

Physical Therapy expertise in home health settings
Experience with orthopedic and geriatric patient populations
Comfortable with EMR documentation using HomeCare HomeBase
Strong communication and patient education skills
Ability to work independently while being part of a supportive team
