Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Assess patients in their homes to determine mobility, function and therapy needs
• Develop individualized treatment plans that promote independence and safety
• Deliver skilled therapeutic interventions to improve strength, balance, and function
• Document timely, thorough clinical notes (Epic experience preferred) and communicate updates to the care team
• Collaborate with nurses, aides and family members to coordinate care and ensure continuity
• Provide patient and caregiver education on home exercise programs and fall prevention
• Maintain compliance with agency policies and complete required competency testing

Qualifications

• Current Physical Therapy license valid in Iowa (License/Certificate Verification required)
• Basic Life Support (BLS) certification
• Home health experience preferred; open to clinicians ready to grow in home care
• Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Must meet required health screenings: fit test, TB screening (2-step PPD or IGRA), MMR, Hep B, Varicella, Tdap, COVID-19 and influenza vaccination as listed
• Willingness to work scheduled weekends as needed and provide patient-centered care in the home environment
• Epic experience preferred but not required

Knowledge & Skills

• Skilled in physical therapy evaluation and treatment in home settings
• Knowledge of home safety assessments and fall prevention strategies
• Strong documentation skills and familiarity with electronic health records (Epic preferred)
• Excellent patient and caregiver education and communication skills
• Ability to prioritize caseload and manage time efficiently during field visits
• Infection control and safe patient handling practices
