Roles & Responsibilities

• Evaluate residents to determine occupational therapy needs and create individualized treatment plans
• Implement meaningful, evidence-based interventions to improve daily living skills and mobility
• Document progress and outcomes thoroughly and accurately to support care continuity
• Collaborate with the interdisciplinary team to coordinate care and optimize resident outcomes
• Engage residents and families with education and coaching to support functional independence
• Meet productivity expectations and follow CRG policies and procedures to maintain quality standards

Qualifications

• Active Occupational Therapy license in New Mexico, or eligibility to obtain one prior to start
• 1-3 years of Occupational Therapy experience (Skilled Nursing Facility experience a strong fit)
• Comfortable working in a SNF practice setting and following facility protocols
• Professional presentation — facility dress code is black scrubs
• Strong communication skills and ability to collaborate with a multidisciplinary team

Education

• Degree in Occupational Therapy or related field (as required for licensure/OT credentialing)

Knowledge & Skills

• Skilled in OT evaluation and treatment planning for geriatric and post-acute populations
• Experience with adaptive equipment, ADL training, and fall prevention strategies
• Proficient documentation skills and familiarity with productivity expectations
• Ability to follow facility policies, maintain punctual on-site presence, and manage caseload
• Team-oriented attitude and strong resident/family education skills
