Shift Type: Monday, Tuesday, Wednesday, 5:00 AM - 5:30 PM

Hours per Week: 36

Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Deliver compassionate, hands-on care to residents at Bloomfield Nursing and Rehab
Administer medications and treatments with precision and care
Monitor patient conditions and report changes to the care team
Collaborate with RNs, physicians, and families to support individualized care plans
Maintain accurate documentation and ensure compliance with facility protocols

Qualifications

Active LPN or RN license in New Mexico
Current CPR certification
COVID-19 vaccination or approved exemption
Strong clinical assessment and communication skills
Ability to work independently and as part of a team

Knowledge & Skills

Skilled in medication administration and wound care
Knowledge of long-term care regulations
Ability to build trusting relationships with residents
Comfortable working in a fast-paced rehab environment
Detail-oriented with excellent documentation habits
