Roles & Responsibilities

• Provide compassionate, direct patient care that improves daily comfort and outcomes
• Administer medications and treatments safely and according to plan
• Perform timely and accurate assessments and document findings in the medical record
• Collaborate with nurses, therapists, and physicians to coordinate patient care
• Educate patients and families on care plans, medications, and safety
• Assist with admissions, discharges, and transitions to ensure continuity of care
• Maintain infection control and safety standards to protect patients and staff
• Support quality improvement initiatives with attention to detail and clinical insight

Qualifications

• Active LPN license in Pennsylvania required
• Graduate of an accredited practical nursing program or equivalent
• Current BLS/CPR certification required or the ability to obtain upon hire
• Comfort working per diem shifts (as-needed) including full 7:00 AM - 7:30 PM coverage
• Ability to pass background check and drug screening as required by facility
• Strong communication, time management, and teamwork skills

Education

• Diploma or certificate from an accredited Practical Nursing program
• Active LPN license in Pennsylvania

Knowledge & Skills

• Reliable medication administration and documentation practices
• Skilled in vital signs assessment and recognizing changes in patient status
• Wound care, ADL assistance, and basic nursing procedures
• Familiarity with electronic health records and clinical charting
• Infection prevention, safety protocols, and regulatory compliance
• Professional bedside manner and effective patient/family education
