Contract Type: Per-Diem

Shift Type: 7:00 AM - 7:30 PM, shifts vary based on facility needs

Candidate Pay Rate: $33 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents in a skilled nursing facility
Administer medications and treatments as prescribed, ensuring accuracy and safety
Monitor vital signs and assess changes in patient condition, reporting to the healthcare team
Collaborate with registered nurses, physicians, and other staff to develop and implement care plans
Maintain accurate documentation of nursing interventions and patient responses
Educate patients and families on health management and post-discharge care

Qualifications

Current LPN license in the state of Pennsylvania
BLS certification from the American Heart Association
Previous experience in long-term care or skilled nursing preferred
Strong clinical assessment and critical thinking skills
Excellent communication and teamwork abilities
Compassionate and patient-focused mindset

Knowledge & Skills

Proficient in medication administration and IV therapy
Knowledge of electronic health records (EHR) systems
Ability to prioritize tasks in a fast-paced environment
Understanding of infection control and safety protocols
Strong interpersonal skills to build rapport with residents and families
