Shift Type: Per Diem / As-needed (rotating days, evenings and nights). Common shifts include Monday-Sunday, 7:00 AM - 3:00 PM; 3:00 PM - 11:00 PM; 11:00 PM - 7:00 AM. Weekend coverage may be required.

Roles & Responsibilities

• Provide compassionate, resident-centered nursing care that preserves dignity and comfort
• Administer medications and treatments accurately and document thoroughly
• Assess residents regularly and communicate changes in condition to the care team
• Support activities of daily living and encourage resident independence
• Collaborate with nurses, CNAs, therapists and families to implement individualized care plans
• Maintain clean, safe and infection-controlled care environments
• Participate in admissions, discharges and care transitions to ensure continuity
• Contribute to accurate clinical documentation and timely charting

Qualifications

• Current LPN license in New York State (required)
• Graduation from an accredited practical nursing program
• Basic Life Support (BLS) / CPR certification or willingness to obtain
• Recent experience in long-term care, skilled nursing, or similar setting preferred (1+ year preferred)
• Up-to-date immunizations and ability to pass required health screenings
• Strong communication skills and a team-first attitude

Education

• Completed LPN / Practical Nursing program and NY LPN licensure

Knowledge & Skills

• Medication administration, including safe documentation practices
• Wound care basics and understanding of skin integrity
• Infection control protocols and proper PPE use
• Familiarity with electronic health record documentation
• Time management skills for fast-paced shifts
• Ability to prioritize, escalate concerns, and follow care plans
• Compassionate resident interaction and family communication
