Roles & Responsibilities
- Provide compassionate social work services to patients and families in need
- Conduct thorough psychosocial assessments to develop individualized care plans
- Coordinate care across multidisciplinary teams to ensure seamless patient support
- Facilitate 5150 evaluations and involuntary holds when patients require psychiatric intervention
- Connect patients with vital community resources and support services
- Advocate for patients' emotional and social wellbeing throughout their healthcare journey
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Two supervisory references required at time of submittal
Knowledge & Skills
- Expertise in conducting involuntary psychiatric evaluations
- Strong understanding of mental health crisis intervention
- Excellent communication and interpersonal skills
- Ability to work collaboratively within multidisciplinary teams
- Knowledge of community resources and support services
