Roles & Responsibilities
- Deliver compassionate physical therapy care across four rural counties
- Conduct comprehensive OASIS assessments and Start of Care evaluations
- Help elderly patients recover from falls and fractures with personalized treatment plans
- Manage a manageable caseload of 4-6 patient visits daily using our point system
- Document visits efficiently with provided tablet technology
- Build meaningful relationships with patients in their home environments
Qualifications
- Current Physical Therapist license in Indiana
- Experience with OASIS assessments and Start of Care documentation
- Strong clinical skills in treating elderly patients with falls and fractures
- Reliable transportation for travel across Noble, Lagrange, Steuben, and DeKalb counties
- Excellent communication and time management abilities
