Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Deliver compassionate physical therapy care to patients across Sacramento County in their homes
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric diagnoses
- Collaborate with interdisciplinary teams to create personalized treatment plans
- Document patient progress and maintain accurate mileage records for reimbursement
- Participate in twice-monthly supervisor meetings to review documentation and ensure quality care
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum 6 months of clinical experience preferred
- Strong communication and time-management skills
- Ability to work independently in home health settings
- Valid driver's license and reliable transportation
Knowledge & Skills
- Home health documentation and productivity standards
- Patient-centered care across multiple specialties
- Mileage tracking and GSA-compliant reimbursement processes
- Building rapport with patients and families in home environments
- Meeting productivity goals of 5-6 visits per day
