Roles & Responsibilities

• Perform thorough speech-language evaluations to assess resident needs and goals
• Provide consistent, high-quality daily treatments focused on functional outcomes
• Manage and coordinate timely discharges with clear documentation and handoffs
• Collaborate closely with the multidisciplinary therapy team to optimize resident care
• Follow Continuum Rehabilitation Group (CRG) policies & procedures and facility protocols
• Maintain productivity standards (therapist productivity requirement: 90%) while delivering compassionate care
• Be present in the building to receive guaranteed hours and to engage with residents and staff

Qualifications

• 1-3 years of clinical experience (as listed)
• Prior SNF experience strongly preferred
• Current credentials and licensure required by state/facility (hold and maintain applicable SLP credentials)
• Comfort with documentation, productivity expectations, and following CRG P&P
• Professional demeanor (dress code: black scrubs) and reliable on-site presence

Knowledge & Skills

• Skilled in evaluations, treatment planning, and discharge processes
• Experience delivering daily speech-language treatments in a SNF setting
• Strong documentation practices and familiarity with productivity expectations
• Ability to collaborate with nursing and therapy teams for resident-centered care
• Familiarity with long-term care workflows and regulations
