Roles & Responsibilities

Collaborate seamlessly with RN case managers to deliver exceptional patient care
Masterfully document patient progress using Cerner, Nate, and Ensocare systems
Communicate effectively with patients, families, and teams to build lasting trust
nimbly shift attention between multiple tasks while maintaining high-quality outcomes
Float where needed to make a meaningful impact across the hospital

Qualifications

Active RN license in good standing
2+ years of nursing experience, preferably in case management
Strong commitment to patient experience and teamwork

Education

RN degree from an accredited nursing program

Knowledge & Skills

Excellent documentation skills in electronic health records
Outstanding communication with diverse teams and families
Ability to multitask and shift focus efficiently
Talent for building trust with patients, families, and colleagues
