Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health therapy to patients in their own homes
Develop personalized treatment plans that make real differences in mobility and independence
Collaborate with interdisciplinary teams to optimize patient outcomes
Educate families on care techniques for lasting recovery
Track progress and adjust therapies to celebrate every milestone

Qualifications

Active Physical Therapy license in good standing
BLS certification required
Epic experience preferred but not required
Completion of skills checklists and competency testing

Education

Degree in Physical Therapy from an accredited program

Knowledge & Skills

Strong home health therapy expertise
Patient-centered communication skills
Familiarity with Epic systems a plus
Proficiency in evidence-based rehabilitation techniques
