Shift Type: Flexible shifts - days, evenings, nights as needed (Per Diem)

Roles & Responsibilities

Provide compassionate, holistic nursing care to elderly participants in a PACE program
Coordinate and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as prescribed
Monitor participant health status and communicate changes to the team
Educate participants and families on health management and wellness

Qualifications

Active Registered Nurse (RN) license in Indiana
Current BLS certification
Experience in long-term care, geriatrics, or community health preferred
Ability to work independently and as part of a team

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Knowledge of geriatric care principles and PACE model
Excellent communication and interpersonal abilities
Proficiency in electronic health records
