Roles & Responsibilities

• Lead discharge planning and coordinate transitions of care to the next level to ensure safe, timely discharges
• Perform utilization review and case management tasks to support appropriate care and documentation
• Collaborate with interdisciplinary teams including social services to create individualized plans of care
• Document thoroughly and accurately in EPIC to maintain compliance and continuity
• Cover and float across hospital areas including Colby and Pacific campuses as needed
• Support needs assessment and DME ordering so patients leave with the right resources
• Apply CMS and regulatory knowledge to case decisions and authorization processes
• Provide patient- and family-centered education to improve outcomes and reduce readmissions

Qualifications

• Active RN licensure (pending licenses accepted)
• BLS certification required
• Minimum 1 year of acute care experience
• Experience in discharge planning, care coordination, or case management
• Comfort working with high patient ratios (days: approximately 1:25) and varied acuity
• Strong preference for candidates with diverse CM experience such as Med Surg, LLOS, Emergency Department or ICU

Knowledge & Skills

• Proficient in EPIC charting and electronic documentation
• Skilled in discharge planning, care coordination, and utilization review
• Familiarity with CMS regulations and regulatory-driven documentation
• Ability to prioritize and manage high caseloads while maintaining quality
• Excellent communication and interdisciplinary collaboration skills
• Experience working with a broad patient age range from neonates to geriatrics
