Roles & Responsibilities

Assess residents’ functional status and design individualized treatment plans
Deliver skilled therapeutic interventions to improve mobility, strength, and independence
Document progress clearly and timely in the medical record to support care continuity
Collaborate with nursing and interdisciplinary teams to coordinate patient-centered care
Educate residents and caregivers on home programs, safety, and fall prevention
Participate in care planning and discharge planning to achieve measurable outcomes

Qualifications

Doctor of Physical Therapy (DPT) or entry-level physical therapy degree (new grads welcome)
Eligible for or licensed as a Physical Therapist in Indiana
Comfortable working in a Skilled Nursing Facility (SNF) setting
Excellent communication and teamwork skills to collaborate across disciplines
CPR/BLS certification preferred (or willing to obtain)

Education

Doctor of Physical Therapy (DPT) preferred; new graduates accepted

Knowledge & Skills

Strong knowledge of geriatric rehabilitation and restorative care practices
Skilled in therapeutic exercise, gait training, transfers, and mobility aids
Familiarity with SNF documentation and care-plan driven treatment
Ability to assess functional goals and translate them into practical, measurable interventions
Good clinical judgment, time management, and patient-centered communication
