Roles & Responsibilities

• Evaluate residents, perform thorough assessments, and set meaningful functional goals
• Develop individualized treatment plans that restore mobility and independence
• Deliver hands-on and therapeutic interventions in a skilled nursing setting
• Collaborate with nursing and rehab team members to coordinate safe, patient-centered care
• Document progress clearly and timely to support transitions of care
• Educate residents and families to maximize functional outcomes and quality of life

Qualifications

• PT or DPT credential as noted in the job posting
• Open to clinicians of all experience levels including new grads
• Comfortable working in a skilled nursing facility environment
• Ability to work daytime schedules and collaborate with facility staff

Education

• DPT or PT (as listed); new graduates are welcome to apply

Knowledge & Skills

• Foundational physical therapy evaluation and treatment skills
• Experience or comfort with SNF patient population and care needs
• Strong documentation and care planning abilities
• Team-oriented communication with interdisciplinary staff
• Patient and family education with emphasis on functional independence
