Roles & Responsibilities

• Provide patient-centered physical therapy evaluation and treatment in a skilled nursing setting
• Develop and implement individualized therapy plans to improve mobility and function
• Document clinical progress thoroughly and in accordance with facility and CRG policies
• Collaborate with interdisciplinary team members to ensure continuity of care
• Meet productivity expectations and contribute to a high-performing therapy department
• Maintain professional presence on-site to receive guaranteed hours and support patient access to care

Qualifications

• Licensed Physical Therapist or Doctor of Physical Therapy (DPT) — credentials consistent with role
• 1-3 years of clinical experience (as stated in the job details)
• Comfortable working in a Skilled Nursing Facility (SNF) practice setting
• Willingness to follow Continuum Rehabilitation Group (CRG) policies and procedures
• Ability to meet a productivity requirement (90%) and maintain reliable on-site presence
• Professional appearance (black scrubs as required by facility)

Education

• Doctor of Physical Therapy (DPT) or accredited physical therapy degree

Knowledge & Skills

• Strong clinical assessment and treatment planning skills for geriatric and post-acute populations
• Knowledge of SNF documentation standards and regulatory requirements
• Good communication and teamwork skills for interdisciplinary collaboration
• Time management to balance caseload while meeting productivity targets
• Adaptability to facility protocols and regional leadership guidance
