Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes
Help patients recover from surgeries, falls, and other conditions with compassion
Create individualized treatment plans that make a real difference
Collaborate with home health and hospice teams to support whole-patient wellness
Travel within a 60-mile radius to bring expert care directly to those who need it most

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Knowledge & Skills

HomeCare/HomeBase EMR experience
Home health and hospice patient care
Treating post-surgical and fall-recovery patients
Managing a caseload of 5-7 patients per day
Independent decision-making in home settings
