Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and other orthopedic procedures
Support geriatric patients managing multiple chronic conditions like CHF and COPD
Document visits accurately using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to coordinate care plans

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Comfortable traveling within the service area and managing a 5-visit daily caseload

Knowledge & Skills

Skilled in home health physical therapy assessments and interventions
Familiar with orthopedic and geriatric patient populations
Proficient with EMR documentation using HomeCare HomeBase
Strong communication and time-management abilities for independent field work
