RN Case Manager

Location: Tuba City, Arizona

Start Date: 06-21-2026

Contract Type: Travel Contract

Shift Type: Monday to Friday, 8:00 AM - 5:00 PM with weekend rotation and call

Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Serve as the supportive contact for high-risk patients across specialty clinics
Coordinate seamless care transitions from admission through discharge
Evaluate patients in the ED for medical necessity and care coordination needs
Collaborate with physicians and interdisciplinary teams to create safe, efficient care plans
Connect patients with vital community resources for ongoing support
Guide individuals facing mental health challenges toward appropriate services
Ensure continuity of care and alignment of services for every patient

Qualifications

Active, unrestricted RN license in any U.S. state, Puerto Rico, or U.S. territory
Current BLS certification from the American Heart Association
Three years of clinical nursing experience including one year in outpatient or inpatient settings
Strong understanding of case management, utilization review, and care coordination processes

Education

Associate's or Bachelor's Degree in Nursing

Knowledge & Skills

Patient assessment and individualized care planning
Care coordination across inpatient and outpatient settings
Navigating resources for patients with complex social and health barriers
Working effectively with culturally diverse communities
