Roles & Responsibilities

• Conduct thorough patient evaluations to build individualized plans of care
• Deliver daily therapeutic treatments that help patients regain independence
• Manage timely discharge planning to support rehab-to-home transitions
• Document interventions and outcomes clearly and accurately
• Collaborate with interdisciplinary team members to optimize patient progress
• Meet productivity expectations and follow Continuum Rehabilitation Group policies

Qualifications

• Licensed Physical Therapist (PT) — active license in good standing
• Doctor of Physical Therapy (DPT) or equivalent clinical education preferred
• 1-3 years of clinical experience (as listed) with strong SNF experience preferred
• Comfort working onsite and adhering to facility dress code (black scrubs)
• Ability to follow CRG policies and maintain required productivity levels

Education

• DPT preferred; PT clinical training or equivalent required

Knowledge & Skills

• Skilled in patient evaluations, treatment planning, and therapeutic interventions
• Experience with discharge planning and facilitating rehab-to-home outcomes
• Familiarity with SNF workflow, documentation, and productivity expectations
• Strong communication and teamwork to coordinate care across disciplines
• Time management and organization to handle a busy caseload
